You refuse to perform the surgery.

After thinking the dilemma over briefly, you decide that performing the surgery will do more harm than good to the woman's life -- a direct violation of the Hippocratic Oath.

You tell the woman to re-evaluate her reasons for wanting the surgery. You could refer the woman to a therapist. This is probably the best decision you could make.
