You are a research interviewer. Today, you are conducting a telephone interview with a respondent named Mrs. Smith. You began the interview at 10 a.m. As the respondent answers your questions, you enter the information into the computer. You are using a special software program on the computer that your employer has provided.

Halfway through your 45-minute interview, the software program freezes. It will not allow you to enter any more information. You close the program and reopen it, but you cannot access the file that you have been working on. The information that you have already entered has not been saved for quite a while, so you don't know whether you will have to start over or whether some of the information has been recorded.

In any case, you cannot complete the interview now. You explain and apologize to the respondent, and ask if you can phone her back at another time. She agrees and you set a new time.

You hang up and look for the form that you must use to record computer problems of this sort.
