Andrew seems to have livened up, and it's true that many of your clients feel better after they've had someone to talk to. Andrew does fall into a risk category that says he should be admitted to the hospital, but human beings don't fall into neat categories.

"There are always indicators, but you never know what a person is going to do," says Dr. Kamal Rungta. "You can make educated guesses that are often correct, but everyone is capable of making a wrong decision."

In this case, Andrew convinces you that he's feeling better and goes home. He doesn't report back to the ward the next day. Later you discover that the police have pulled Andrew off the streets and charged him with harassment and disturbing the peace. You could have prevented this from happening if you'd admitted Andrew.

"Psychiatry requires very sophisticated decision making," says Rungta. "Even when you're tired or late for a meeting, you have to put every effort into thinking about what's best for a patient."
