You're a safety supervisor at a big brewery. At this brewery, more than 300 bottles are filled per minute and any stoppage in the production lines is cause for concern with the management.

You're doing a routine check of the production line when you notice a large amount of broken glass among the bottles heading to the packaging area. The bottles themselves are sealed, so there's no danger of the glass getting into the bottles.

Broken glass is common on the production line and workers wear protective gloves for this very reason. Still, it looks like a lot more glass than usual.

The production line at this brewery hasn't been halted for a record 22 years.

Should you shut down the line and have the glass cleared away, or should you go down to shipping and warn the employees?

"Safety officers are often seen to be making a big deal over nothing," says Dave Dietsch, a safety inspector in Maryland.
