Lunchtime supervision is one of your daily duties as a teacher aide. Today (Monday) at 12:15 p.m., a student ran up to you to tell you that one of the other students, David Cooper, was hurt.

You found that David had scraped his knee quite badly. The injury was about three inches long by two inches wide. David explained that he tried to jump off of the monkey bars and that his foot caught on the bar. He lost his balance and managed to fall onto the gravel, where he slid on his knee.

"Part of the job as a teacher aide is to write up reports," says Dykeman. "Sometimes during lunchtime a student may have an injury. If it is treated, no matter how minor, we need to write something up."

You let the other playground supervisor know that you will be taking the child inside to clean and dress his wound. Once you have completed this, the bell rings and lunch is over. The student goes to class, and it's now time for you to fill in a report.

Write up the incident in an easy-to-understand yet concise report for the office to have on record in case any questions arise.
