The up-and-down blood sugar levels this patient is experiencing are dangerous, but the doctor thinks he can control them without making any dietary changes. After all, this doctor has shown over and over his belief that nutrition has only a small role in health care. You don't intend to push your luck and make your working relationship awkward.

Because you didn't say anything, this patient's blood sugar chart starts to look like a roller-coaster. His health continues to decline. This doctor remains uninformed about the importance of nutrition in the recovery of patients.

You should always offer your best recommendations for the patient. This is true even though you don't always have the final say, says dietitian Daniel Tisi.

"In the end it's not your butt on the line, it's the doctor's butt on the line," says Tisi. "So you have to just say (to yourself), 'If they're putting their license on the line and they want to do this, then that's their choice.'"
