Real-Life Communication -- Solution

Here's what you could write:

A 58-year-old male patient, Manny Cooke, came to the Heart Institute Clinic. He was asymptomatic. No chest pain, dizziness or shortness of breath was detected in his initial physical exam.

The patient volunteered to undergo an echocardiogram because of family history of coronary disease. According to the results of the cardiac ultrasound, there is an embolus in his left ventricle, resulting in infarction.

I recommend that Mr. Cooke undergo immediate surgery to restore normal blood flow to the heart. Referred to cardiologist on the same day.
