Real-Life Decision Making

Besides being knowledgeable about medicine, the physiatrist must also be a skilled juggler. Dr. Timothy Doherty says juggling is part and parcel of his daily work.

"When you're dealing with people who are sick, you're dealing with the fact you're trying to help them. But you're also dealing with the pressures of dealing with their families, and the other health care professionals. So it's really the challenges often of juggling multiple different pressures that you have to try and get your job done."

For Dr. Jennifer Yao, one of the biggest challenges is determining whether a patient has "rehabilitation potential." In other words, whether or not that patient would benefit from a rehabilitation program, even if the benefits are slow in coming.

She offers this real-life scenario. "I may be asked to see a 35-year-old man who was recently injured in a car accident. He suffered a severe brain injury but is now alert and able to walk around independently. However, he has poor attention and memory and is unaware of his cognitive deficits. He is impulsive and inappropriate at times."

His family doesn't think the effort required to treat him is worth the trouble. He seems keen to do the work, however.
