Real-Life Decision Making -- Solution

You recommend that the man be sedated so the machine can take over ventilation needs.

Normally, sedation is not the first option. You should have first tried to adjust the machine to match his breathing.

His breathing difficulties could be caused by many factors. He may have collapsed a lung. Maybe he hasn't had a bowel movement for several days. Perhaps he's been told his wife died in the accident.

You also expose the patient to other potential dangers with heavy sedation because it depresses the natural defense mechanisms.

Respiratory therapists will normally "try and change the mode of breathing on the machine or tailor the machine to the patient," Allan Shemanko says. "That's sort of the art of respiratory therapy."
