You try to adjust the machine to match the patient's breathing.

This is the real-life decision made by respiratory therapist Allan Shemanko. He says heavy sedation is not the first choice.

"If you sedate somebody, you also depress a lot of their defense mechanisms like, for example, coughing," he says. "If you have an impaired cough reflex, you're more prone to develop pneumonia."

Therapists will normally "try and change the mode of breathing on the machine or tailor the machine to the patient," Shemanko says. "That's sort of the art of respiratory therapy."
