Ruby, who has been pregnant for a few weeks, has come for a visit to her obstetrician because she is experiencing some abdominal pain. As the physician assistant, you ask her additional questions about her health.

It's possible that you might be able to assist her without having to send her on to the physician in charge. You ask her questions and find that she has experienced some bleeding. She has also had some dizzy spells.

These symptoms immediately make you think that Ruby might be at risk for an ectopic pregnancy. An ectopic pregnancy happens when the embryo begins to grow outside the uterus. This can be an extremely serious situation. If the pregnancy is inside the Fallopian tubes or the ovaries, a rupture could cause the mother to bleed to death.

You order an immediate ultrasound. The results confirm that Ruby does indeed have an ectopic pregnancy.

You now have a decision to make. Do you immediately refer her to the obstetrician? Or because you know about ectopic pregnancies, do you continue with Ruby under your care?
