How about writing a letter reminding the doctor how important this documentation is and the risk she is placing the hospital in? Inform her of her "on hold" status until the matter is cleared up. Give a copy of this letter to your supervisor. If Dr. Burg does not follow through, your supervisor will have documented course for taking action.
