You write Dr. Burg a letter suspending her until all documents are in order.

This is harsh, but it would work. She is putting the hospital at great legal risk by continuing to practice without all documentation being current. Suspension should get her wheels in motion for taking action.

Be sure to mention all attempts to remind her by phone of the upcoming expiration of the document, and those calls placed after expiration. Point out that two different people in your department placed these calls.

Ask her to contact you if she needs help obtaining current documentation. Give a copy of your letter to your supervisor for future action, if necessary.
