You decide to pull the tooth.

You explain all of the options and their consequences to the patient, and he still insists on having the tooth pulled. You do just that. The patient leaves the dental office with a gap between his teeth and a grin on his face, having saved a few dollars and rid himself of his toothache.

This is the real-life decision made by dentist Don MacRae.

MacRae says that in every diagnostic decision you make, the patient's own decision comes first. If they want a tooth pulled, even though you don't think it's the best option, you have an obligation to do what they want. You can offer your best advice, but you have to let the patient decide.

"The patient knows the best thing for them," agrees dentist Hor Tak D. Zung. "Anyone with a medical background would know the patient should have the final say on what happens."
