You revamp the unit.

You figure too many jobs are at stake, and you can't help but feel hospitals have a moral responsibility to provide this service to the people who desperately need it. You trim equally from all departments to boost the mental health and substance abuse center and keep the hospital within budget.

For two years, everything is fine. But in the third year, insurance companies stop covering drug addiction therapy and cut mental health coverage. Your hospital starts to flounder with a unit that is no longer able to make money. You are forced to close the unit after it has racked up a huge debt. The board blames you for the loss.

Six months later, your contract is canceled and you're out of work.
