You wait to do the hysterectomy after the baby is born.

You decide this because the patient wants her baby to be safe above all else. She will worry about herself later.

Jaira isn't a doctor. It is up to you to counsel her and tell her that the baby can survive outside the womb at eight months. If you act immediately, you may have a chance to cure Jaira's cancer. "Prompt treatment can often cure the cancer," says Dr. Frederick Stehman.

If you decide to wait, the infant will be born on its due date, but the mother will be very ill, with a much-decreased chance of surviving her cancer.

"Some of our toughest decisions are when the unexpected happens, or there is a conflict between basic tenets or principles by which we live," says Stehman. "We want to preserve life and alleviate suffering. Some of our hot topics have to do with pregnancy termination, in vitro fertilization, adoption...and even cancer care."
